Hill Park, Bexley

Hill Park Outreach Service is a service supporting people in Bexley and the surrounding boroughs who have a learning disability, an autistic spectrum disorder or a mental health problem. HPOS supports people to live independently and helps them to gain new practical and social skills, whether living in their own homes or with family.

The HPOS office is based in Erith next door to the support living service and provides a range of services and assistance devised to improve the quality of people's lives and experiences.  These services include outreach and preventative support, day & vocational opportunities, supported living, socialisation, personal Care, inclusion and support at home.

We provide self-directed support by working with an individual to devise their personalized support plan and opportunities for those who want to develop special skills to assist in independent living.
