Community outreach Aspens provides community outreach care for people who are living in the community, either with their families or independently. As with all of our services, we aim to help individuals to develop strategies and life skills such as budgeting, cooking and cleaning, to enable them to gradually reduce the requirement of formal support and live independently. After a person is diagnosed with learning difficulties, complex needs or being on the Autism spectrum, Aspens will work with them and their families or carers to create an individual support plan. We then take the lead in the process of negotiating this plan with the local authority to secure funding for them. We offer personalised support in a range of living environments and situations: People who live at home with their parents or carers People who live in supported accommodation People who live independently in the community People who live in a residential care home Manage Cookie Preferences Chat with us, powered by LiveChat
